Poisoning Facts...

Despite being recognized as a poison for nearly 50 years, ethylene glycol remains readily available and continues to be fatally ingested. The most common sources of ethylene glycol are automotive antifreeze, engine coolants and hydraulic brake fluids.


The lethal dose of ethylene glycol for a cat is one teaspoon, for a medium size dog is 1-3 tablespoons, and for an adult is usually between 2 tablespoons - 1/2 cup (30-100ml).

A progression of toxic effects upon ingestion can be roughly divided into three stages.

Stage 1: From 30 minutes to 12 hours after ingestion: Symptoms include depression, intoxication, nausea, vomiting, uncoordinated movement, and convulsions.

Stage 2: From 12 to 48 hours: Metabolites produce severe acidosis with compensatory hyperventilation. Calcium oxalate crystals are deposited in the brain, lungs, kidneys, and heart which can all fail resulting in death.

Stage 3: From 24 to 72 hours: The direct toxic effects of ethylene glycol metabolites in the kidneys can cause acute renal failure, acute tubular necrosis (kidney failure), bone marrow suppression, coma and death.

If treatment is initiated early, prognosis is excellent; however, a disturbing proportion of patients are admitted at a late stage to hospitals that are not capable of performing analysis which identifies ethylene glycol toxicity on a 24-hour basis. Therefore, rapid treatment is often prevented because of a delayed diagnosis, which may result in fatality.

In the 2002 annual report of the TESS data by the AAPCC, 6,077 exposures to ethylene glycol were reported in the United States, resulting in 40 deaths and 254 near-fatalities.
